Dependents of members have the same health coverage as the member when enrolled in the program. Certain documentation requirements must be met when adding a dependent to your coverage.
Student, Handicapped and Other
Dependent children are allowed to maintain enrollment up to age 23 (up to age 25 for students in the military), if they are full-time students. Dependents
who qualify as being students, handicapped or “other”, must
be recertified each spring and/or fall through the recertification process. The Dependent Certification Form (PDF 15 KB) must be completed and submitted to the agency Group Insurance Representative (GIR) (PDF 327 KB) by the due date in order to maintain active enrollment in the program.
Adult Children
Public Act 95-0958 provides for the extension of health, dental, vision and prescription coverage for three new categories of dependents, referred to as 'Adult Children [PDS, 122KB]', who may or may not have previously had coverage under the State of Illinois Group Insurance Program. These new categories are in addition to the current dependent categories of Student, Handicapped and Other. Adult children added under these new categories are not eligible for life insurance coverage.
Domestic Partner
Members may be eligible to enroll their unrelated same-sex Domestic Partner
in the State Employees Group Insurance Program upon experiencing an eligible qualifying change in status.
Members should contact their Benefits Office to verify eligibility for participation.
Members interested in enrolling their same-sex Domestic Partner can access an enrollment and information packet [PDF, 265KB] that
includes an Information Sheet, Domestic Partnership Affidavit, Domestic
Partner Enrollment form and a Termination of Domestic Partnership Group
Insurance form. |
