Opt Out with Financial Incentive

Non-Medicare State Employees’ Retirement System (SERS) annuitants who are currently enrolled in the State Employees Health Program may elect to opt out of the health program and receive a $150.00 financial incentive per Public Act 94-0109. Members interested in this option must complete the Opt Out Election Form with Financial Incentive (PDF 20 KB) and submit to the SERS Group Insurance Representative (GIR) (PDF 327 KB).

Annuitant Waiver to Become a Dependent

Upon receiving a retirement annuity, or during the annual Benefit Choice Period, an annuitant may elect to become a dependent of their spouse for health, dental and vision coverage and continue life insurance coverage as an annuitant per Public Act 93-0553. Annuitants electing this option must complete the Annuitant Waiver Form (PDF 9 KB) and submit to the State Employees’ Retirement System Group Insurance Representative (GIR) (PDF 327 KB).
